400 large breaches in first half of 2025

400 Large Breaches so far in 2025: HHS has announced that during the first six months of 2025, there were 400 "large" breaches (500 or more individuals affected) added to the HHS "Wall of Shame."  

Breaches happen, so the fact that you have one doesn't mean you did anything wrong,  But it might: show me a copy of your most recent security risk assessment.  If you can't, then you have done something wrong, and OCR might decide that your failure to do a good security risk assessment is the "cause" of your breach, and punish you accordingly.
