Another OCR Patient Access to PHI Settlement Agreement: For the 55th time, OCR has slapped the hand of a HIPAA covered entity that did not give an individual access to their own medical records.  This time it's Azul Vision of California, which failed to provide a patient with access to her PHI until 2 years after she initially asked.  More information here.
