Oops: The University of Pittsburgh Medical Center sent out a mailing to potential donors, and included on the reply card an identifier that included the recipient's social security number. Probably not a big problem in itself (although one should refrain from sending social security numbers in the mail), but the numbers were visible through the cellophane window in the outgoing mailing envelope. Still not a high risk, but a possible one, and an easily avoidable one.
