And Right Here at Home: Ft. Worth's Texas Health Harris Methodist Hospital is
notifying some folks who were patients 20 - 35 years ago that their PHI might have been breached, after a box of microfiche records that was sent off to be destroyed instead turned up in a Dallas park. Names, (presumably old) addresses, birth dates and some social security numbers were in the data. Because it was microfiche, it was not shredded on site by Shred-It, the contractor/busines associate.
This is not the first time I've seen something like this. Be very careful choosing your shredding contractors.
