Swedish Medical Center: The Washington state health regulators
audited Swedish in Seattle and found some deficiencies; nothing major, but one of the problems involved patients' PHI in areas where it was viewable by others. Not a huge deal, but a little reminder to review how your institution handles PHI and take steps to minimize the likelihood that inappropriate folks might view it. Charts behind the nursing station desk, whiteboards out of sight, minimal patient information on doors or outside rooms, etc.
