Want a great resource for documentation requirements? There's been a lot of discussion on the ALHA HIPAA listserv about the funny way different segments of the Privacy statutes require the covered entity to maintain something in writing or allow the covered entity to require the patient to submit their request in writing, while in other areas documentation is not required. In that discussion, John Cody, Assistant Counsel to the New York State Office for Technology, pointed out a very useful chart put together by the New York State Central HIPAA Coordination Project outlining the things HIPAA requires you to document. Best of all, it's annotated to the HIPAA privacy regulations. Check it out.
